Application Information

APPLICATION  PROCESS:    Each student must complete an application (online -  available in February) and submit.   The application must have all required information that includes writing a short essay why you are interested in attending this Health Care Career Academy.       The Reference Form must be filled out by your teacher or counselor and submitted by them.  It will be placed with your application.   

 A Social Security number will be required for tracking purposes for the grant if you are accepted into the Academy.  Please DO NOT SEND this number with the application.  You will be advised of a secured website to submit this once you are accepted.    

A CDIB (American Indian role number) if available will be asked to be submitted for demographic information for grant purposes.  However, it is not required that you are of an American Indian race nor have a CDIB number to be able to attend the Academy. 

APPLICATION CHECKLIST:   Review checklist before submitting your application.  This includes: 

  • • Application completion (make sure all blanks are filled in) 
  • • Email addresses and phone numbers are legible and correct
  • • Brief essay on why you are interested in the Academy
  • • Have your teacher or counselor complete and submit the Reference Form 

Application Deadline:     Applications are due by  March 24, 2014.    Applications received after this date will be accepted for alternates .   Alternate applicants must be aware that they may be notified on short notice.   Due to a students’ inability to attend the Academy (illness or emergency), they may cancel at the last minute and an alternate will be called.
